I was looking to get more info on this.

Im sure there is a way I just don't know the path.

I would like to set up a PDF file that has all information on there.

Such as:

Name:
Address:
Phone:
Etc.

I want to set up the file in PDF where they can download and not be able to change the document but have the ability to edit and add their information in the required fields.

I have tried searching on how to do this but I am lost.

What programs can I use or need.

If you're using Office 2007, SP2 includes their PDF creator. Additionally, there's CuteWriter, which prints a document to PDF.

why not just create a word document using the forms, and then lock it for forms. Then they can only fill in the stuff that you request, and can't change the master document at all.

You will need a PDF creator that allows you to do forms. Adobe's "Pro" version is super expensive so I hope that maybe one of these will have what you are looking for.

As for signing, I self-sign PDFs using an MD5 hash. There are several you can download from the internet.
