I was just recently asked to take a picture of a man who is writing/publishing a book.
I won't go into the details, but what forms do I ask him to sign?
A shared rights form? An adult model release form, even though he is taking my photo for his book? (I did ask for credit somewhere on the jacket cover.)

What should I get from him to make sure I get credit?

Have any of you run into this? I was kind of blind-sided with his request and my lack of experience is now glaringly obvious....

You'll want a model release from him, and then to write him a license for the image. I typically sell rights to use on their website, the jacket, and for other promotional materials as long as I have a photo credit, and I write all that into the release. I do not give copyright -- just a limited license for use. You need a model release from him to use it on your site and for your own promotion.
