You shouldn't have to pay if you can show it was obvious he completely missed the mark. Anyone can have a communication issue with a client, but if I came back to you with ideas and had completely missed the mark, I'd be going back to the drawing board and eating the time spent. Now if it was a case of the client changing direction, then we'd have to have a little talk.
If he was willing to do another round, it might have helped to show him other logos that were similar in style to what you were looking for.
To answer your first question, we typically get a solid understanding of what you are looking for before we get started. And this is why we do multiple designs, because no matter how well you explain it, we never know for sure exactly what's going to make you happy. Your version of "elegant" may be a little different than mine. By broadening the scope a little bit it helps us feel you out, and it helps you better visualize the thoughts that were in your head and get a better idea of how it actually looks on paper. But no, other than verbally discussing your needs, nothing gets shown up front. It takes time to research and come up with solid ideas/designs worthy of presentation.
