Contract: Rights..

I am researching contracts to use for general portraiture for my business. I am seeking common examples of what might go in the following blanks regarding Grant of Rights. I've never done this before so forgive me for lacking the information.

If you need an example of the type of job, how about model/actor portfolio work (paid - not trade out) and how about wedding photography. Are they the same? Different? What might you not grant the client rights to do?

Grant of Rights. Upon receipt of full payment, Photographer shall grant to the Client the following exclusive rights:
For use as __________________________________________________ _____________________________________
For the product, project, or publication named __________________________________________________ _________

For the following time period or number of uses______________________________________________ ____________
Other limitations __________________________________________________ ________________________________
This grant of rights does not include electronic rights, unless specified to the contrary here:

in which event the usage restrictions shown above shall be applicable. For purposes of this agreement, electronic rights are defined as rights in the digitized form of works that can be encoded, stored, and retrieved from such media as computer disks, CDROM, computer databases, and network servers.
