You need to be more specific. There are model release forms for TFP shoot, some for minors, some for paid work... All are different in some way.

And BTW, I know for a fact that there are many threads on this subject as I have posted my own releases for folks to customize. Perhaps a quick trip on the search box will lead to some results for you...

If you search for model release it brings up 18 pages of threads. Make sure to check your spelling when searching.

I had a similar question a while back about the same time as someone else. In that thread the book "Business and Legal Forms for Photographers" was recommended. I bought it off Amazon.com for ~$25. It comes with a CD containing the forms that can be edited in your favorite word processor.

The nice thing about this book is not only are forms presented to you to use as is, the author, Tad Crawford, explains each form and its use. Tad also takes the time to explain what the legal terms mean both to you and the client. Finally, Tad provides a checklist of negotiating point to use when working on the contract. As is explained in the book, the forms are all written to the photographer's advantage and then the negotiating points are there to help YOU win the job.

May I also recommend Legal Handbook for Photographers.

If you go to the bookstore here at TPF, both these books are listed. Oh, and the forms book has a fourth edition coming soon. :)
