The Premium depends on how much equipment you are insuring (yes I got my equipment covered under the same policy...includes laptop, portable studio, camera and all other gear that I take to locations.) My Premium is $500 a year with a $500 deductible. For each installment they charge an extra 6 dollars and they offer 1, 2, 3, 4, 6, or 10 installments.
The companies they use are Travelers, The Hartford and the last one I cannot recall.

same with me. initiated through usaa, and it's actually through travelers. mine is 2 mil per occurance, 4 mil max per year though with a $250 deductible. same $500/y, billed out as like ~$130 every quarter. mine didn't matter what equipment i had, it was all covered and didn't need to be listed out.
however i did have a run in with trying to get a dropped lens replaced. basically they would pay the cheaper of: 1. retail cost of an equivalent lens, 2. the market value of your lens, 3. repair of the lens. and that's all after your $250 deductible. so for my cheap tamaron 2.8, i basically wouldn't get squat because it was only ~$200 to repair the lens, which is less than my deductible. now on an expensive lens (like a nikon 70-200 2.8) i'm sure the replacement ins would come in handy...
