Help needed with order form --- - 03-07-2011, 04:14 PM

I have an opportunity to photograph a small college's graduation service. I'll be taking the photos as the graduates receive their diplomas. I would like some help designing a form...or not recreating the wheel.

I'll need a form that will be placed in the student's graudation package so that they will know how to order their pictures.

Please feel free to share your experience and/or forms for an event of this sort.
