There's no tricky legal language about it. Basically you write two sentences.
You (your business entity) gives them (the band) the right to use (describe the photographs) in particular contexts (CD cover, promotional materials for the band, etc...) In exchange for this right, the band agrees to give you credit on the CD (describe with some particularity where the credit is going to appear, who will be credited, what size type if you want, etc...)
Hope this helps. The best contractual language is the language that most closely represents the understanding reached by the parties. The more specific you can make it, the less likely you are to be surprised.
