Can someone help with release forms?

I did a photoshoot for a friend of my wife last night.

Since this is all for free, I would like to give here some kind of release to help her when she goes to walmart or any other photolab to get prints made.

Any advice or help would be great.

I am not sure what others do, but we use lightscribe burner that prints that they are allowed to make the prints of images on the disk.

I just started selling cd's instead of packages of prints so I bught this forms

This is the form we have used for many years. We print it on our company letterhead and add in the photographers name, the subjects name, and the exact uses we are giving permission for i.e. personal photographic prints, or one time use in a yearbook, or website and business cards only. I hope this is what you were looking for.

Thanks Don. The form sample you provided is just what I needed. Took photos of friends last Sunday as a gift. Also a way of getting back on target in using my equipment. Hope and pray your health is better. I've been off the forum for some time. It's time to get back on the horse.
