I did a shoot for a agency and have invoiced the agency for the image usage. I initially had sales tax included on my invoice, but they said it's for resale, so they will be collecting the tax.

Do I need to get a resale form from them for my records for each transaction, or is this something we do at the end of the year?

You need a form for the first job which lists their resale certificate number. Keep that on file if the tax man ever comes to your door. I copied one from one of my vendors and just changed it to our studio name. You should be able to pick one up at any pro camera store or album or frame supplier.

Here ya go Andrew: Exemption Certificate

You need the Exemption Certificate signed by the company you sold the images to. The Resale certificate is for purchasing product that you intended to resale.
