I've got the Lowepro Lens Case 3 that I'd be happy to sell you. It's just sitting in my closet right now.

While I don't think you'd go wrong with the thinktank, a photographer I met in OK once bought a decent belt system at the sporting goods department at Wal-Mart. It was cammo, but offered protection and access. I don't know part numbers, but if you're at any similar store, you might just take a look at the options.

My 100-400 came with a case. I thought the 70-200s did too.

Yes, my 70-200 came with a case, but it doesn't allow you to have the hood on.

The Think Tank LC75 works great with the lens. I prefer using it with the Pro Speed belt. You can then easily use other Modulus components.

I started using the Tamrac Slingshot, #9 I think. I can put a large lens, necessary items, and it stays on my back when I need it out of the way. When I need quick access, I just spin the slingshot around and open the top and I have everything at relatively chest level. It works well for me when I am going too far afield to carry a full backpack.
