Scott the band will do their recording and come up with what is called a Master recording and then they will choose a pressing plant to reproduce the CD's, there are a couple of these plants that are national in scope in the Plano/Dallas area, well the plant will also do the CD sleeve and any insert printing (part of their price quotes) and they will combine whatever case that is chosen (most use the plastic butterfly cases), the sleeve the CD the CD holder and any inset together on their line and then shrink wrap it. You just submit all of the graphics and print info to the plant.

Of course they use outside printing shops but they use a handful of shops in the DFW area that give them a good rate vs you going to that shop directly because of their volume of business done. Bands that even self release go this route!

Here is one of the bigger CD shops

These guys are smaller but I bet knowing their location that they should have good customer service-(have not dealt with them)

These guys are well respected here in Dallas
