So I have noticed lately that my photos are VERY noisy..

I am shooting at low ISOs and do not understand why...

Here is one example... Shot at f/5.6 1/50 s Iso 400 55-200@55MM



Please let me know what i can do to improve it!

It's hard to tell much from the resized web photo, but iso 400 isn't really that low. Can you post a 100% crop?
Are you bringing up the exposure any in post?

Maybe it's my office monitor, but that doesn't look that noisy to me.

I don't really see noise, but I would say this:

Don't shoot that lens wide open.
Shoot faster than 1/50th s for a sharper pic
Buy a 50mm 1.8 for better quality pics.
Buy a steamer to get the wrinkles out of your background.

All that will help your general shooting. As for noise, the examples you have shown aren't showing much.

As far as I can tell - the only thing you are doing wrong with respect to noise... is calling something noise that I don't call noise. I think you are good!

Tweaking colors in post processing can give you some problems.
