Need Youth Sports Contract

Does anyone know where i can get a Contract from youth sports i was ask to shoot 5 - 6 yr old basketball. Just need some to protect me and the kids

Let me get this straight. You were ASKED to photograph a team and you need a contract? For what? You will have no problems if you use pre-pay order envelopes and deliver WHAT you said you would deliver WHEN you said you would deliver it. That's it.
It already appears they trust YOU. Honor that trust.

I think we don't do business with a hand shake anymore because we tell each other we don't trust each other.

Under promise and over deliver! But back it up with a written contract.
