I changed mine a little from the original I posted. I like this one betterI lightened it, soft focus, unsharp mask and adjusted saturation levels to the model only.

Ok.. this looks like a rough, urban kind of shot to me.. so I tried to make it look like it was old and uncared for....

I set black and white points (white on the wall behind her, black on her top).. to balance and lighten..

I added an "aged newspaper" effect to it overall.. 10 years, I think..

I textured it with crumpled paper...

Finally brightened 20% and contrast by 10%

Here's my entry. The mood of the photo felt desolate to me so I applied a black and white action and tinted the photo after adjusting the exposure via the curves tool. Then I dodged her face to bring it out of the shadow and cropped the photo because her right foot was bothering me...just didn't seem like it fit into her shoe. The result seems more personal and has a lot of emotion... at least to me it does.

Kinda had the same idea and feel to the orig. image as Brad did so I went for the old photo look also.

Applied an "aged film action"
Made several layers adding some overlays of the image in different tones to give some lines through the image and around the edges. And made a blur layer which I toned down to suit my needs.

I actually did about 4 different versions of the image and could not figure out what I wanted to do with it, so I just picked this one and went with it.

Well, mine looks similar to a bunch of others. Ah well. Here's my 5-minute version.

Levels & contrast
A little brightness added to just the face
Channel mixer to brighten the colors
Gaussian blur, less on the face more on the edges
High pass filter

What really stands out to me are the eyes and lips...so, I decided to accentuate those features. I did feel that the dark look worked well with the setting, so I decided to really bring out this and came up with the following:
