None here. I just use what's included, and then adjust the layer opacity until the effect is where I want it.

^ same for me. I'm sure I'm not using it to it's full potential, but I just haven't sat down and really learned all about it.

I could see using it for LR now. In PS, I have to either open in PS or export first, then run the plug in...But What I do is make a layer, adjust the opacity, then erase the portraiture layer from eyes, hair, teeth etc. so that those areas are sharp but skin only is smoothed.
It works based on a color range- anything within a certain color range gets smoothed. Sometimes hair is included in that range, as well as other surroundings.
edit: I think they have an education discount or something. Otherwise it's pretty stiff, but it's worth it.

I always have it generate a transparency mask so I can add back in texture (noise) and fade it to varying degrees - it's also much easier for me to edit the mask if I already have a starting place (ie ctrl+click then click add layer mask).

pretty much the same here.
i copy out the face, (or other skin parts as needed) depending on the model i just use the regular presets. then i adjust the opacity so i can regain some of the texture back (or not, depending...).

Treat different areas of skin appropriately. I have presets for face, arms, legs, and body. All tuned to leave the right amount of texture based on the part of the body. i.e. legs need less large than faces, etc... Never let it mess with hair. Of course YMMV.
