Some programs strip it by default.

Yeah depends on the workflow and how the JPG is created as to if the Exif is embedded

When you "Save for Web and Devices" in Photoshop CS5 (and previous versions), the default is No Metadata. You have to change it to save the metadata. I never use that unless I'm actually saving an animated gif. But, in older versions, before I knew it stripped EXIF, I used to save using the Save for Web feature. I don't even think you had the option to save the metadata in the older versions of PS.
