Here's what I do.
Repair blemishes and touch up color etc.
Duplicate Layer
Apply Gaussian Blur about a 10 or so depending on image size
Add a Layer Mask to this layer
Select a soft edge brush
Carefully paint away the eyes, eyebrows, nostrils, lips, jewelry, neckline on shirt etc.
Now paint away the rest of the image that does not need softened, like hair etc
Drop opacity of this layer until things return to a more natural look.

i use digital skin - followed with portraiture as well...depending on how much has to be done..

Todd, that's a great skin softening. I think it’s a big improvement over the original without looking like a plastic doll.

I'm going to give that a try when I get home. Thanks for the step by step technique

I use Pro-Retouch from the Boutwell's totally Rad Action Pack. Works great and does not look fake. Here is a link to a video tutorial on the Pro-Retouch action.

I was doing it all manually but now I'm using Portraiture more.

I use Portraiture. It's just one click, and it works really well. After that, if there are still issues, I will use the airbrush tool or the patch tool.

David, Todd, both of you have great results, both ending in acceptable natural looking texture.
David, your model has a strong resemblance to the actress on that show, Cold Case.
