I'm not answering as to who the client was. That you'll have to wonder about.

Roy, I actually did that with Microsoft Digital Imaging Suite. I was playing with the program and found a nice little "shrink and bulge" filter.
I shrunk her in places, and bulged her in other places.

I think you overenhanced her arm when other parts were done. Now she has Popeye arm.

Tom if you are doing a body reconstruction photoshop surgery maybe you should enhance a little in the back so the image looks more balanced.
