Football Picture Templates

I may be shooting team pictures for a team that missed picture day. I hate these things, but the team mom's are requesting some kind of graphic almost like a baseball card. Here's an example...



Besides making your own, does anyone know where I can find a photoshop template to buy or use for free? Any suggestions would be appreciated.

mpix has some very basic layouts. i used layout for 8-9 year old girls soccer and it turned out really nice.
