Creating mail forms in DW

I created my old website in Front Page. It had a neat little option that enabled me to create a form with drop down menus, radio buttons, and text boxes that would capture information and email it to me when a potential customer clicked the button at the bottom of the page.

I can't seem to figure out how to replicate this simply in DW8 unless I write a lot of code (not going to happen). Does anyone know an easy way to do this?
